I have very high arches which means a room-consuming orthotic. Also have a gouty toe--these are the most comfortable shoes I have ever had. There is an extra littl hole above the metal eyelets, this allows a final tight lacer that allows me to keep the rest of the foot loose enough, but keep the shoe securely on my foot. LOVE IT
